Job Description

Social Worker (MSW, LCSW)

Location: Campbell Hall Rehabilitation Center.

Employment Type: Full-Time.

Position Summary:

Campbell Hall Rehabilitation Center is seeking a licensed Social Worker to coordinate and provide comprehensive social services that address the psychosocial, emotional, environmental, and discharge planning needs of residents. The Social Worker works collaboratively with the interdisciplinary team to enhance resident quality of life while ensuring compliance with federal, state, and facility regulations.

Education & Licensure Requirements:

  • Master’s Degree in Social Work (MSW) from an accredited New York State School of Social Work.
  • Current New York State Social Work license and certification in good standing.

Experience Requirements:

  • Prior experience in a healthcare or long-term care setting preferred.

Essential Functions & Responsibilities:

  • Implement departmental policies and procedures in accordance with current regulations and resident needs.
  • Complete timely resident admissions, assessments, MDS documentation, social histories, care plans, and progress notes.
  • Participate in interdisciplinary care planning and resident care conferences.
  • Assess discharge potential, develop discharge plans, and assist residents and families with adjustment and transition planning.
  • Provide individual counseling and casework services to residents and families as needed.
  • Assist residents and families with financial and medical assistance matters, including Medicare, Medicaid, Social Security, pensions, and other benefits.
  • Advocate for resident rights and assist in the resolution of grievances and complaints.
  • Serve as liaison with the Ombudsman program.
  • Support and attend Resident Council and Family Council meetings as requested.
  • Conduct staff orientation and continuing education related to resident rights, elder care needs, quality of life, and abuse, mistreatment, and neglect prevention.
  • Provide consultation to staff regarding residents’ psychosocial needs and social service concerns.
  • Maintain current community resource files and departmental manuals, records, and documentation.
  • Conduct annual PASRR (PAS SAR) reviews as required.
  • Consult regularly with the Social Work Consultant.
  • Investigate, document, and follow up on resident and care-related complaints.
  • Serve on facility committees, including Interdisciplinary Care Plan, Resident Council, Department Managers, Family Group Meetings, and Quality Management.

  • Must demonstrate legal eligibility to work in the United States.
  • Ability to read, write, and speak English clearly and effectively.
  • Strong communication, assessment, and documentation skills.
  • Ability to exercise sound judgment and make competent decisions regarding resident care and department operations.
  • Flexible, dependable, and cooperative with staff across all departments.
  • Tactful, patient, and compassionate when working with residents, families, and visitors who may be distressed or challenging.
