Requirements:

• Active CMT, CNA/M, or LPN/LVN certification/license.
• Experience in Long-Term Care or Skilled Nursing (preferred)
• Strong attention to detail and medication safety practices.
• Ability to work independently and as part of a team.
• High level of integrity, dependability, and compassion.
• Passion for caring for long-term care residents and skilled rehabilitation patients.

Responsibilities:

• Administer medications via approved routes under RN supervision.
• Maintain medication carts and secure, organized storage areas.
• Obtain approval before administering PRN medications.
• Perform non-narcotic medication counts at shift change.
• Monitor medication supplies and coordinate reorders.
• Accurately document medications on the MAR.
• Obtain and record vital signs, including apical pulse.
• Observe and report changes in resident condition.
• Assist with resident care, including CNA duties.
• Communicate effectively with nursing staff, providers, and care teams.
• Maintain professionalism with residents and families.
• Work efficiently in a fast-paced environment with frequent interruptions.
